
NHS 2 NHS Concern Reporting
It has been noted by CCG's locally that care homes and domiciliary providers do not raise many N2N concerns ( NHS to NHS Concerns).
This is a process that gives organisations who provide NHS care ( including independent providers) the opportunity to identify and feedback where there are concerns relating to patient care delivered by other organisations who also provide NHS care.
This allows the CCG to identify any trends and to take appropriate actions as required.
These concerns need to go specifically to either Shropshire or Telford CCG depending upon who is the appropriate commissioner.
For Shropshire there is a bespoke form(to download in the folder) to use and in Telford & Wrekin there is a note explaining where the concerns should be sent.
